Why do plants rot? how can you tell before its bad? and how do you prevent it?
I've had a hard time keeping sag from dying and some of my micro sword has dead leaves.(micro sword is new)

Take a few minutes and read my Guide. Your plants are starving to death. And you need to crank the CO2 up a bit. But that won't help till you get some food in the tank for the fish.

Nitrates
Phosphates
Potassium

yep. I noticed the same. theres no mention of Any benifical ferts goin in aside from the Iron. Can I ask why you are dosing Excel when you have CO2 going in?
You need to add Micro and Macro ferts. So grab yourself a bottle of seachem comp and as stated above N,P,K
you got lots of light and CO2. but you need to finish the puzzle.
