So I bought a banana lily (Nymphoides aquatica or N. mexicana, unsure) about two weeks ago from the LFS. The aimed for TDS is 150 ppm (tap water w Prime + RODI water to soften it). I give 12 hours of light a day, which the amazon sword plant (Echinodorus osiris) seems to love, and I feed heavily. I'm not seeing roots of any sort, nor leaves from the tubers. I do water changes anytime the ammonia or nitrites spike, but ideally want to aim for 50% ish water changes in the 20L every week (10 gallons per week, which is more like 80% but whatever).

When should I start looking for leaves to appear? What about roots? I bought just the tuber, and so far nothing. When should I start worrying about the plant's overall health?

Make sure it's not mushy, tubers can take a while to start growing. Keep an eye out for the tap roots to show, could be a couple months before leaves start pushing out, but the roots should start showing within a couple weeks.
