I have a 307, and there's a single piece that attaches both intake and output hoses to the top of the filter. It's a unique mechanism, and I dig it (as opposed to my eheim 2213 with no frills at all). I like that you can adjust the flow or just stop it completely for water changes without unplugging the filter. And it's simple to quick release both hoses without making a mess.

To answer your question, they do both have to come straight up out of the top for at least a few inches in order to accommodate the quick release and the connectors. From there, I suppose the hoses can go anywhere. You could plumb in PVC angles or whatever you need if you get creative (it comes with ribbed tubing which is plenty flexible). The only real requirement is that the intake from the tank has to be above the filter.
