Peppered Cory died

So I have noticed the last few days my Cory's eyes have been picking out really far and to day she died . I have also noticed that at times she would swim to the top of the tank fast and back down . Any ideas water is OK all other fish OK

a lot of algae in the tank? how much oxygen in the tank? parameters? heater? filter? tank specs basically <----? might have been co2 if you have that added

I don't have water parameters right this second but not much algea some on the rocks , has heater and filter , no Co2
