I have had this plant in my tank for awhile but cannot remember the name of it maybe you all know what it is.

Looks like pearlweed to me

To propagate this plant do I just pull a few roots up and move or do I trim it and just stick the stems directly into the substrate?

Yep that's pearlweed. You can do whatever to it, and it will grow. You can trim it at the base, it'll regrow. You can plant any size stem, it will regrow. You can float it at the surface, and it will regrow.
