mystery plec disease

Hi.
This is my first time posting, so please forgive my ignorance!
I have a plec in my tank (and have had him for nearly 4 years now!), normally he is healthy and happy. However over the last 4 days, I have noticed he seems really lethargic, spending all his time lieing on the bottom of the tank...he moves so little, the only way I know he is still alive is when he moves his eyes.
He also has developed white spots on his back and head. I have had white spot before (a long time ago) and the spots look too big for that.
I'm pretty sure the mark on his head may be a burn (he normally likes hanging on the glass behind the heater), but I don't know what the other marks are.
Any help/advise on what could be wrong with him and what I can do would be greatly appreciated! I'm really worried about him to be honest.

Could be heater burns, looks like it to me anyway. I am not sure what medicines are available to you. I would definitely treat with something against bacterial infections like Maracyn or Maracyn II.
You do not have any other plecos in that tank correct? If so that could be the other pleco rasping on him which is not good, need to remove the injured one to a hospital tank for healing.
Heater burns are also a possibility, my dwarf gourami has fallen asleep next it it twice ad burnt his lower lip. I finally caged off the heater using plastic mesh from the art store and fishing line (with plenty of gap between the mesh and heater so it never touches/melts).
No, I haven't got another pleco in the tank.
The only other fish in the tank are a loach catfish, a snail and 4 free swimming fish.
