Just received dry ferts for EI dosing. Can someone help me figure this out?

Okay, so I received my little sample packs of EI dosing of

k25o4, csm+b, kh2p04, iron 13% and kn03

I'm wondering how much of each goes into my tank

40gallon breeder with one 39watt t5 bulb sitting right on the rim of my tank.
