I am using powerhead with top filter box. I haven't touched my filter box (other than changing the mechanical filter every week) for 3 months now.

Should I flush it with clean water now? How often to do this?

Depends on the bio load of your tank, could be 1 month, could be 1 year. If there is a thick slime forming, good sign you could rinse it out
