46 gallon tank
GW is so bad i cannot see more than 3" into the tank.

Just bought a TurboTwist 9w UV sterilizer and put it inline with the inflow of my RENA XP2 canister filter and restricted the flow down to about 150GPM.

How long should it take to start noticing a difference in the GW breakout? s 9w too small?

The 9W should be OK. You should start seeing results within 2 days.

should i run it 24/7 forever or do most turn them off once the GW is gone??

Some keep them on 24/7 but the UV bulb has a limited bulb length. Some people only turn them for a short while right after a large trimming (which tends to induce GW) or after a WC.

I'd keep it off to save bulb life.
