a photo would help (just to make sure it's not a nutrient thing). Loaches would make a 'v' shape hole. Mine did that for a little bit but they stopped. I think they just do it for fun.
Fish eating holes in plants
I am told to blame the Clown Loaches for eating holes in my swords and other broad leaf plants. Are there any other fish in my sig that could be doing it?

I've watched my Angels tear round holes in a sword
I would bet on the yoyos before the clowns! Clowns may uproot plants but I have witnessed yoyos stripping leaves from some plants.
