(Emergency) is this what I think it is?!?!

It looks like a bacterial infection which is usually secondary infection after a possible injury or as you suspect ich. I don't see the white spots I would expect to see on the fins and rest of the body that make it pretty easy to spot. Does he seem to be rubbing up against rocks, plants glass etc.? If it were fungal I would expect it to look white and fuzzy but that's not to say it not, its hard to tell from the pic

Just to add you want to remove activated carbon or purigen from your filter when dosing medications.

I am in the midst of an ich outbreak in my tank. The outbreak was caused by an ammonia spike from damaging my bacteria in the filter. For the first time, I decided to just raise the temperature to 86 degrees F (slowly). The results have been much better than any I have ever had from medication. Without knowing much about your fish, I'd suggest you look up treating ich in this manner. It is much easier than medication without all the issues that come with staining your tank and copper based meds.

The simple explanation is ich can not reproduce above 85-86 degrees (with one very rare exception)...and the temperature reduces the life cycle of ich to 2-3 days. Then you hold the temp up for another week to ensure that everything is clean.

Should I raise temp from 84?
Do I do a water change?

I would leave it as is and as @Coralbandit said monitor it. Since you have already started dosing what I'm assuming is malachite green you may as well continue and if the infection was caused by parasites or its a fungal infection it should clear up. If it continues to grow and get worse then you will want to conclusively identify the cause before treating as often times you can do more harm then good by just throwing different medications at it.

I agree, definitely not ich. Looks like the beginning of the hole in the head disease to me (Hexamita). I think Lymphocystis would be protruding on top of the skin as opposed to what looks like a hole? Any other symptoms like stringy white poop or anything?

Agree that it is NOT ich. Ich is common but also easy to spot after seeing it once. One clue is the individual spots are not big enough to show in a picture from that distance.
No, I would not raise the temp but instead try to keep things as great as possible in the tank. Ideal would be too move this fish to a QT where there would be several advantages. One is the fish could be less prone to problems from other fish, the owner can observe the fish better and the treatments will cost far less. When faced with wanting to treat but not having a real idea of what since this might be bacterial or fungal, I like to use more of the wide range products like Furan-2.
But it is also very hard to get the precise disease spotted. Good luck.
