Dwarf Baby Tears separated from Substrate

My dwarf baby tears has been planted for almost a year and has managed to completely cover my substrate, but I've noticed over the past few months that it's come loose from the substrate and is "bulging" in a few places. Any one dealt with this before? Any ideas to fix it? Should I put more soil on top of it or perhaps sand to weight it down?

Have you pruned and replanted at all? Most likely, in the areas that are growing thick, the top layer will prevent light from reaching the lower layer, causing it to die, and then lift off.

What Darkblade said was spot on. If you haven't trimmed it in a while it can cause the carpet to lift.

Thanks all. Yes, I've never trimmed it, so that's clearly the problem. The edges around the tank seem to still be attached. I'll try to figure out a way to replant it and then give it a trimming to prevent this from happening in the future. Fun...
