As aquarist said, basically all guesses unless there is a photo of the plants to look at. Even a photo of whatever is left will tell a story.
Sounds like your plant probably died from a lack of light. A single T8 bulb on a 20g tank isn't enough.
Also, potted plants are a waste of money, they cannot be grow in the pot and must be removed and replanted in your tank.
