Anubias Nana and Java Fern melting.

A few days ago, Ive noticed my java fern develop a browny black patch. Beleiving it was just a dying leaf, I left it. I didn't have time to trim it off since I was leaving for a trip, so I let it be. Two days later (yesterday), I come back home and notice that the brown has spread to the anubias right next to it.
Today, the brown patches are melting away with the plant.

I pretty much know its not algae ... more like Zombie plant necrosis? :icon_roll

My tank is a 20g high, Medium low light (2 T5 14w tubes), Fert once a week with Flourish, No CO2 (recently took off DIY because it was causing horrible algae problems).
This is the best picture I could take: http://i.imgur.com/Tg8m7.jpg
There is some Green Spot algae (left from the horrible DIY incident) on the front of the anubias, ignore that

Any idea what is causing this?

I had the same symptoms before on my anubias and could not for the life of me find out exactly what causes the brown spots. But I did find a fix to the problem. The symptoms went away after I increased the ferts by 1.5 timesand increased co2. Although I could not salvage the damaged leaves, but at least it is not spreading to the other leaves.

I am not entirely sure if it is ferts prob, light prob, co2 prob or plant disease. But I suspect that it has to do with ferts, light and co2 for the most part. If I would to make a guess, it would be co2.
