Weird worm in shrimp tank

Hi guys,

i don't know if you guys could see it. But i found out a weird worm on the nose of my shrimp. It looks like leech but much smaller & doesn't have triangle head like planaria.
Can someone please tell me what this thing is & how to get rid of it? I'm reallyyy freaking out.
