Drop in O2 levels

Last week I had an issue where I woke up the morning (10 A.M.ish) after a water change only to find that I lost 2 cardinals and an angel fish. The rest were all swimming at the top gasping for air. I immediately dropped the water level and spent 20 minutes twirling my tweezers around trying to re oxygenate the water. After about an hour the fish started moving away from the top though you could tell they were working hard to breath. Things seemed to normalize over the course of the day but I did end up losing another cardinal.

That part that confuses me is that I didn't do anything different from my routine water changes. Filled it up to the spot that I always do. Is there anything other than not getting enough surface gas exchanged that could have caused the drop in O2 levels?

It's a 90g tank with two spray bars.

Ambient temp change, weak needle valve, end of tank syndrome maybe. Could have been the water change. Drastic change in temp, if city water: water plant could have just treated it with something. Just throwing out thoughts

Do you use CO2? What is your water change schedule/recipe? How old is the tank? We need more baseline info!

No CO2. Tank has been up since August though all of the plants came out of a well established tank. It's fairly heavily planted. Water change once a week at about 15-20%. Dual canister filters.

Stocking:
Angel x2
cardinals x13
rummy nose x6
congo tetra x4
SAE x4
oto x4
hovering zebra loach x8
