Chronic fish death?

My 450GL planted tank is setup for a little over 1 month.
The tank is loading with 110 Tetra and other small fish.
There is an extra large biological filter and I change 20% of water every other day.
The water parameter seemed fine.

However, I continue to lose a fish or 2 every few days.
Some of them seemed to have no external symtons.
They appear to be eating and suddenly died the next morning.
Previously I removed 4 of my fish with Columnaris and 2 were breathing heavily.
They are now doing fine in the hospital tank treated with antibactiria.

My question is, what can be done and should be done?

What are your readings at? (pH, nitrates, etc)

PH: 7.2 ~ 7.5
NH3: 0
NO2: 0
NO3: 5

Any larger temperature shift?
Chlorine, Chloramine, heavy metals?

Temp around 23 - 25 and very stable.
KH: 7
No heavy metals since we are using deep well water in the mountain hill far from polution.

Maybe the tank still hasn't complete cycled?
