Plants suddenly dying, very weird

My Hygrophilia corymbosa suddenly lost all of its leaves over the course of two days. It was thriving, and now, I'm pretty sure it's dead. Now my Staurogyne repens is melting too. What is could cause this?

Without a lot more information it isn't possible to even guess what is wrong. What size tank, what lighting, what substrate, what fertilizing, any CO2, how long has it been set up, etc.?

Thanks, Hoppy. I should know better.

This is a 58 gallon tank with a 4 x T5Ho light fixture, pressurized co2, miracle grow soil capped with black diamond, Up and running for well over 6 months now.

Things that have changed recently:
1. Started EI dosing dry ferts about a month ago and have seen a significant boost in growth.
2. Ran out of CO2 a couple of days ago and went without it for 2 days before I could refill the tank.

The problem started with the Hygrophilia and it took less than a day for all of the leaves to melt and drop off. There are none left now

The Staurogyne Repens is melting too, but not as fast. Both were looking great less than a week ago.

I did a big water change and I think it has stopped the melting, but really, I just want to understand what is happening.

Here's a picture if that helps.

Measured ph, it seems low, around 6.4. I have been having a hard time getting my CO2 levels up. Maybe that is the problem? I use a cerges style reactor with about 6 bps.

Stumped here, please help

I have a 55 w/ cerges. I found (since plants have grown in) I need a bps of
12-15 to get the co2 levels right. I only use 2x54w t5ho but I have it suspended 2 ft. above the tank.

Is your light hanging? My suggestion is more co2 combined with a slight decrease in light intensity
