Ugh did the dumbest thing ever.

I did a bunch of cleanup in the tank last night with a huge water change. I cleaned both intake tubes from my HOBs and cleaned the the plastic inlet mesh on my powerhead, put it all back and turned it all back on and went to sleep.

Woke up this morning and like I always do before I leave for work, I look at my tanks. Its then that I notice that I forgot to put the plastic mesh back on my powerhead. It ended up sucking in one of my bloodfin tetras and one of my cute little ottos. Gah dumb mistake. Poor fishies didn't deserve such a cruel death

I did that once it sucked up one of my angelfish. Not a good feeling.

Sorry for your loss, I feel your pain. I've had the stupid end basket get knocked off the HOB and awful noise as two of my mollies got sucked in. Now I make sure that thing is on so tight and always check it. It still kills me to think about.
