Help me figure out what defficiency

I am having a lot of trouble recently with my plants. I never had them growing great like most of the tank I see here, but they certainly weren't dying like they are now. I dose daily with my macros/micros from greg watson. My tank size is 72gallons, I have an eheim 2217 & Fluval 304 filtering it. I also have an 18watt UV, Hydor inline heater and DIY Co2 reactor w/ a 20lb. Co2 tank. I can't figure out what is causing my plants to die. I do weekly 50% water changes also.

*Edit - I have 260 watts of lighting over the tank. The substrate is 3M colored quarts on top with shultz aquatic plant soil underneath it.

S,T,Th,S - I dose:
KNO3 - 6grams (13.4ppm)
K2SO4 - 11grams (18ppm)
KH2PO4 - .75grams (1.92ppm I just raised this from 1.4ppm because of green spot algae)
Magnesium - 14grams (5ppm - Not sure if it's needed but it can't hurt)

Every Other Day I dose 30ml of CSM+B+Fe (1.25 TBS in 500ml water)

My tap water recently changed from
PH - 8.2 or 8.4
KH - 7 or 8
GH - 9

Now my tap water is:
PH - 7.2
KH - 3
GH - 5

My tank water is:
PH - 6.5
KH - 3
Gh - 13

Here are the pics of my symtoms:

I am a little confused here. When you say you dose 6grams; do you mean you dose that each of the 4 days, or do you dose a TOTAL of 6grams over the week span (i.e. 1/4 of that each of the days)?

IF you are dosing that much each of the days, you are obviously extrememly over dosing NO3 and K. In fact you are throwing in 114ppm K per week and 59ppm NO3 per week. Based on that, I assume that you are dosing over the span of the week, otherwise you need to cut back

IF you are dosing throughout the week, then you *may* not be dosing enough NO3 or PO4. But, you are the person with the testkits, so obviously stick with your gut feeling if you have plent of each. How old are your testkits though? It doesnt hurt to dose up to 1/4t per KNO3 3-4x per week per 20g's, and then you wouldnt have to dose K2SO4 at all (plenty of K from KNO3).

You dont need to be adding that much Mg. I dose an extra 2 ppm Mg each water change, but even that is not needed. GH of 5 is plenty.

You could up your CSM dosing by 50% too. My calculations show that you should be dosing 36ml 3x per week minimum, but if you were to dose 125% (as GW recommends) then it would come out as 45ml 3x per week.
