Need help and advice

Well guy, I need some help and advice please. I currently have a 55 gal tank that been up and running for about 14 months. Everything been going great until a few months ago. The problem is at all my new leaves leafs on my jungle val, anub, and swords grow deformed and have holes in them. I'm at a loss on what to do.

Tank specs:

Lighting - 2 48' HOT5 - one 6,700k "daylight" and one 12,000K "sun wave" bulbs --- and I run them 7 hours a day
Fert - root tabs and daily flourish dosing
CO-2 - none
PH - 6
ammo - 0 ppm
Nitrate - 5 ppm
Nitrite - 0 ppm

Weekly 25% water changes

Please let me know what other information might be useful

What type of fish are in the tank?

10 blood fined tetras

1 rubber nose pleco

And some pics:
Go to the ferts section of the forum and take a look at the EI dosing sticky, it looks like to me that you are having a deficiency of macro fertz, the flourish (I am guessing you are only using comprehensive?) is just micros,
