what is this nasty creature in my tank!

Planted 7.5 gallon with sandy substrate that is just over 2 months old. Only shrimp, random snails and one otto. Found this guy under some buce today. Looks like a roundworm/nematode of some kind? Not sure. Later saw it sticking its tail out of substrate. Do I need to treat this tank? See attached pic.

That would make sense as I see blood vessels pumping. Was able to get it out with a Turkey baster. Hopefully the only one in there. Thanks!

Depending on what specific species it is, it actually doesn't hurt leaving them in. They turn the substrate periodically, and they make a nice live snack for any fish. You want to stick to aquatic species in that situation. Lumbriculus variegatus is a good example.
