Name this creature

Hello!
I always vacuum clean my substrate when changing water. Today an ungly worm came out of the substrate. It is very thin (half a mm), red and 4cm or 2 inches long. Is this a parasite? I've had two neons bloat up and die a couple of weeks ago but I haven't found the culprit..

Ugh, I had one of these in my 5 gallon a few weeks ago. I came downstairs unable to sleep at around 2am and flicked on the tank light to enjoy some quiet tank time and nearly screamed as I saw that exact size shape and color *thing* wriggling on the glass. I wrangled it out with tweezers and threw it outside to freeze to death.

I have not yet determined if it was some kind of giant planaria or a leech.

Regardless, I reach into my tank with more care since I encountered.....it. however, no deaths experienced of any major livestock of betta or chili rasboras. Nerite is ok and my dearth or pond and Malaysian trumpet snails are ok, too.

Looks like a standard aquatic worm (Limnodrilus hoffmeisteri) to me. Pretty harmless albeit unsightly.
