ID Plant Problem

I bought this plant at the local Petco several months ago and I'm not sure what it is. For the first month or two it flourished and sprouted new leaves. But for the last few months there has been no new growth and the leaves seem to be fading and showing some decayed. Can anyone help me ID the plant and make suggestions as to its decline? It's in a low-tech 75 gallon and I fertilize twice a week with liquid Flourish.

its a kind of sword - Echinodorus... but i cant be more specific ( maybe cordifolius or grandiflorus

Looks like echinodorus cordifolius. Radical sword. My experience is they are heavy root feeders, need rich substrate for good growth. They also really like growing emersed, out the top of the tank, with their feet wet.
