Im no expert -- but, it looks like dwarf baby tears (Hemianthus callitrichoides)?
Its very pretty in tank.

I've never seen Baby Tears growing upwards like this, it usually grows horizontally.

drop the dwarf, I think it's just baby tears - Micranthemum umbrosum
