Rotted roots?

Added some crypts about a week ago. Today i noticed a few melted leaves so i went to pull them, when i did this i accidently uprooted the plant (lutea). When i uprooted it, i noticed the roots looked off and were rotted. I removed the rotted roots, cleaned em off, and replanted them. I havent checked the roots on my wedntti, or echindorus. The main bit the roots grow out of seem fine, and it seems to be attempting new roots so replanted after i cleaned em off. Any ideas what might cause this? Only things i can think of are, too close to a root tab? Or not enough substrate? I'm using a finer very light weight pool filter sand.

IME, most of the existing roots will rot away, to be replaced by new ones. When I plant crypts, swords and such I cut the roots to ~2", just enough to keep the plant in place.
