Unhappy Tank going downhill - Need help please!!!

Ok so I am pretty new to the planted tank - only in it for about 7 months now, and am still very much learning everything. The last bit my plants have been dying, all the roots going brown and the plants that I take out of the tank smell bigtime.

Here is my setup:

90 Gallon
Coralife CF - 2.88 WPG
Injected CO2 with Rhinox 5000
setup for about 7 or 8 months
PH - 7
Ammonia-0
Nitrite -0
Nitrate -40!!!!!
KH-4

I am currently adding:
Flourish
Magnesium sulphate - 1/3tsp
potassium sulphate -1/3tsp
potassium nitrate -1/3 tsp
Something called KN -1tsp
All every 2nd day

I am sure I am not overfeeding as I am very careful and didn't have this problem until recently (Like last month or two) the only thing that changed is I got the Green slime algae which I know can kill plants, I treated with Erythromycin and got some more bottom feeders...this algae is now gone.

My tank isn't overstocked either I dont think:
2 Discus about 2.5" or so
2 German Rams
10 Neon tetras
2 Killifish
4 Cories
2 Clown loaches (Each about 1.5")
2 Otocats

My substrate is Flourite and about 3" deep or so.

Any ideas as to what the problem is? Like I said I am very new to this, I feel like maybe I am missing something & Everytime I get opinions they all cross each other.

I would REALLY appreciate any help befor I have to throw out all my plants!!

Im with turbowagon here with all the stuff you and I dont see where you have any phosphates going in.

Whats this green fuzzy stuff thats growing in my tank.

Oh really???I think I thought that's what the KN was... I will swing by the local shop and pick some up!

With PH at 7.0 and KH at 4 you need more CO2, assuming your test kits are accurate.

Also, what kind of plants do you have in there? And what are the symptoms of death? Brown disintegrating leaves? Pinholes? Yellow leaves?

Well!

You Nitrate in my opinion are way to high, I will going around 7 to 10

Personally I will try to lower a litle bit you Kh/PH ó> 20mgl/6.8 but itís not probably a priority.
What are you other parameters:
Mg
Ca
Po4
Fe
Temperature?
water circulation?

it could be the light, that may explain why your plants arent taking up any nitrates.
