Since dead fish, I need your opinion

I've seen many examples but they don't apply real well to me and I just lost 7 fish and yes I've read the guide. I tried Chucks calculater but everything isn't in there. The Firtilator I can get into.

Soooo Please in your opinion...

Does this look ok to you or do you think it might be to much?

4 ml of KNO3 which was diluted 3 tsp to 250 ml water,
12 ml KH2P04 which was diluted 1.5 tsp to 250 ml water,
1/2 tsp K2S04 dry mixed with a little water and added to the tank, (I'd rather dilute with water and add to the tank but how much?).

Off days for Trace.
12 ml of CSM+B which is diluted at 3 tsp to 250 ml water,
Iron Chelate mixed at 3 tsp to 250 ml water but don't know how much?

Id rather add everything as a liquid because it is just easier for me.

Would I add this at water change and the same every other day?

Do not add any iron at this time.. you wont need it. Iron is a luxury, not a necessity. There will be enough iron in the trace mix.

Joe, I am far from a fert expert, but the potassium Sulfate looks high.

I personally use 3/8's Tsp in my 125.
