Getting a little fed up. Can you help me diagnose these holes?

I've been keeping 30ppm of co2, 10-20ppm nitrate, 1-3ppm phosphate and what I believe to be 10-20ppm of potassium for a couple of months now. I measure e everything daily.

However, some of my plants are still getting holes that turn huge and start yellowing.

My calcium and magnesium is fine, and I've been dosing with CSM + B and keeping the iron around 0.5ppm.

Light is 40-50 par for 8 hours a day at the base. Co2 is at 30ppm when lights comes
on, and co2 shuts off when lights shut off.

Wtf is going on? I did do two very strong hydrogen peroxide doses to knock out some long standing BBA. Would that cause plants to die like this? I'm leaning against it because this was happening before that.

usually no with a peroxide treatment. with an excessive peroxide treatment the leaves would lose green color going yellow then progressing to transparent and falling off. what is your cleaning routine and water changes? im thinking regular water changes/ larger water changes and cleaning of the tank would help alot.

Is the bacopa the only plant thats suffering or is this happening with all the plants?

Yes it can if you hit the leaves with a concentrated dose it can cause precisely that.

Almost every plant that has flat leaves get the holes and then yellow.

I'm just wondering if these look like a specific deficiency in case I'm missing something.

Also, I do about a 40-50% water change once a week.
