40 ppm NO3 too high?

I checked my water parameters today and got some higher readings. I did increase my dosing so it was somewhat expected.

NO3: 40 ppm
NO2: 0 ppm
NH3: 0 ppm
PO4: 3 or 4 ppm

Are either NO3 or PO4 too high? I'm EI dosing with weekly 50% water changes. My lighting is medium light.

I had a African dwarf frog die and a Purple Harlequin Rasbora jump the tank in the last week. The frog was at least a couple years old. Not sure if it's coincidense or if the water parameters are related.

I've been dosing this way for 3 months.

I would reduce by half. 20 ppm nitrate and 2ppm Po4 should be good. 50% water change
