Whats wrong with my Anubias?


I have had this Anubias possibly lanceolata or congensis. I have had it in this spot for a very very long time (10+ months). A few weeks ago the tips of the leaves started to turn black and spread through the rest of the leaf.

Any Idea what this is about and how to fix it?

I dose Seachem Flourish weekly to bi-weekly.
Substrate is flourite/gravel + espoma palm tone (in gel caps stuck into the substrate) + azomite (in gel caps fingers length down). I haven't added added substrate ferts for about a year.
