ID please

Hello, i was wondering if any of you would be able to ID this plant
I bought it from a local guy under the name "Siamensis" but he didn't really know what it was.
Sorry for the bad photos, i was using my phone, and i haven't been taking as good of care of my tank and plants as i should be
leafs show sign of deficiency and have fallen off at the base, roots come out from the bottom parts of the stems

Grown in high-ish light (dual T8, diamond fixture) with CO2 and dry ferts. substrate is gravel capped miracle gro

You need more potassium in tht sucker. The leaves are and indication, that an the led size seems smaller than it should.

@ HybridHerp: i dose a 1/2 teaspoon of mono-potassium phosphate and potassium sulphate every second day in the tank it is in (55 gallon)
i am not really sure what i should be dosing though, any help is thanked
