Water change fish death

Anybody had fish death during a water change this weekend in Cincinnati?

I had one and another tried to jump out of the tank. I'm wondering if the waterworks add extra chlorine or chloramine.

I'm not in Cincy but over the past two weeks I noticed a noticeable change in my water parameters (alkalinity, hardness, TDS)...which did effect my pH and therefore the amount of CO2 and oxygen in the water.
