Advise on algae growth

hey guys,

This is my set up,

18 gallon tank,
Pro flora under substrate,
DIY Co2,
weekly 25% water change, after water change 10 ml of ferts (iron and pottassium)
daylight and power glow 15 watt tubes on for 8 hours a day plus relfectors.
Have annubius, dwarf hair grass, java moss, some other types of swords.

I'm having the problem with yellow algea appearing on my glass and yellow algea on my gravel,
I dont know the ph or nitrate levels.

Any ideas please?!
