What is causing these brown spots and what is the Plant called?

I have a Fern type of plant do not know the name but getting brown spots.

I use the EI fert way and dose everyday.

Also a tad of Excel forced co2 1 bubble a second plenty of light.

Any hints?

Thanks.

This plant looks familiar and non aquatic, but I can't recall its name. Have you considered this plant might not be an aquatic plant? You'll learn that not all plants sold as aquatic are aquatic. Next time you go shopping for plants carry an aquatic plant ID book with you.
