I find it a bit strange that the nitrates are 0 in a discus tank. Do you do a lot of water changes? Can you double check that with another test kit? Local pet stores usually do it for free.

Basically by improving conditions long term (dosing ferts, possibly getting CO2). You can also use some peroxide and a syringe (for feeding medicine to baby's from the pharmacy) to help it die off....

The cause of S. repens melt is still a mystery as far as I am aware. Not sure what might trigger it. It has never happened to mine despite the various deficiencies I've thrown at it.

Your second plant picture looks normal as well. Let the plants settle in for another two weeks or so, they should keep growing well. It can take a while for plants to recover from a bit of neglect.
