Is anyone successfully keeping a heavily planted tank without any fauna? If so, are there any special requirements for doing so?

Do the spots look like salt, if so, probably Ick. If the spots are large and look fluffy then it's probably fungus.

I'd recommend keeping all your plants in place...

I'd recommend keeping all your plants in place with the water level 2- 3 inches. Keeping the plants in place should give the babies something to hang out on/in so if your gravel does shift they won't...

Looks like a Rotalia, maybe Indica or Rotundafolia(sp?). Maybe they had to tie the stems on the wood or put some type of material, like a pad of coconut husk for it to root in.

Are their shells ice cream cone shaped? If so, those are Malaysian Trumpet snails... very beneficial for keeping the substrate clean. I love my MTS, wish I had more!

You may have been dealing with Planaria killing your shrimp. Fenbendazole works well to eradicate them and Hydra. You could use Potassium Permanganate for the snails.

How big is your tank and what kind and how many Cichlids do you have in the tank? Have you paid attention when the lights are out? Cichlids are known for being aggressive towards their tank mates.
