Just dose prime for the full volume of the tank as you start filling. It shouldn't be an issue. I've been doing it for a few years now with no issues and I know many others who do the same.

You can have a nice looking tank without it. CO2 typically becomes necessary with higher light levels. If you stick with easier low light plants you'll be fine.

Shouldn't be a problem. The sand may work itself down into the cracks of the substrate underneath over time though so make sure to add enough on top.
