Pin holes are a result of potassium deficiency. Being diligent about that makes a TON of difference.

Buying bulk ferts from someplace like aquariumfertilizer.com or other sites was a good choice...

I've been having the EXACT same thing happen, though recently it's been a swath of them. How often are you dosing ferts? I suspect it could be fertilizer related in my case, so I need to back off.

Some of the holes remind me of what happens in my tank if I don't dose enough K+. Have you tried upping just that nutrient? It's one of the few that you can overdose without issue IRC.
