Really, if it's only one shipment, it would be worth your time to track down a reputable trans-shipper that deals with fish (look on Aquabid in the betta section, lots of trans-shippers!)

They already have their import/export permits and are used to dealing with the USDA.

I know that up here (canada) it is pretty easy to get the permits and do shipping. But actually being able to do the customs paperwork is an adventure in and of itself. The US's paperwork is no better really. Unless you plan to learn the HS Tariff system and the import regs surrounding fish and so forth, you'd probably be better off just paying someone else.
