Swim bladder issues can come about with injury OR disease. This sounds bacterial and is usually (although not always) related to water quality. Try a gram negative or broad spectrum antibiotic such as Maracyn 2 or Maracyn Plus. First, d/c the erythromycin and run the carbon over night along with a water change, then begin treatment with the new meds.
