Is it a fairly new sword in your tank? It kinda looks like an emersed leaf dying off. If not, do you use root tabs or some other ferts to get the nutrients to the roots?

it's been about 3 weeks and yes, using root tabs

Either emersed->submerged transition, or possibly nitrogen deficiency. Iron deficiency has typically been whole-leaf yellowing for me with swords.
